 Job Description:  Serves as one of the Local Intellectual and Developmental Disability Authority’s representatives of the Community Living Options Information Process (CLOIP) Program to provide standardized information, explanation, and educational materials of available community living options to all individuals ages 22 and above who are residents of Brenham State Supported Living Center and their Legally Authorized Representative (LAR).  For persons interested in community placement in a specific location, coordinates and facilitates arranging sample community tours prior to annual meeting.  Submits results of tours, explanation of living options, and interview results with individuals, LARs, or advocates to individual’s Inter-disciplinary Team.  Based on assigned caseload of up to 85 individuals, attends annual planning meetings of all assigned individuals to discuss Community Living Options: requires routine travel to Brenham State Supported Living Center.  Ensures information is shared with the individual’s designated LIDDA.   Also provides crisis prevention and management, monitoring, assessment, and service planning and coordination to nursing facility residents who are eligible for Preadmission Screening and Residential Review (PASRR) IDD Service Coordination within our seven counties service areas.  Ensures access to medical, social, educational, and other appropriate services that will help an individual achieve a quality of life and community participation acceptable to the individual, based on a Service Plan (SP), monthly monitoring of outcomes, and at least quarterly convening of service planning team.  Negotiate and facilitate the array of services and supports needed to address the individual’s personal goals and desired outcomes as identified through the Service Coordination Assessment and Service Planning Team. Ensures planned services and supports are implemented and appropriately documented, provide education in community living options, and will advocate and ensure consistent interactions with nursing facility and other agencies as necessary. Provides required monthly face to face (and collateral) contact monitoring of persons on caseload and documents activities as required by Texas HHSC-IDD Services and MHMRABV.
EducationRequires a Bachelor’s or advanced degree with a major in a behavioral, social, or human service field, including but not limited to psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human development, gerontology, educational psychology, education, and criminal justice.
Experience:  Prefer experience working with individuals with IDD; must be able to work independently as well as with a team. Must be computer literate, requires excellent verbal and written communication skills, requires excellent organization skills, including the ability to meet rigid timelines and manage multiple priorities.  Good organizational and time management skills are a must.  Knowledge of service coordination principles, objectives, standards, and methods; knowledge of Medicaid waiver program services and non-Medicaid waiver program services; skill in determining individual needs assessment and the ability to respond to changes required; ability to comprehend and implement program goals.
Hours of Work:  Monday – Friday, 8:00 a.m. – 5:00 p.m.; some flexibility maybe required occasionally to meet the needs of the program/person in services
License/Certifications:  Must successfully complete Agency and TAC require IDD service coordination training within allotted timelines as applicable.  Valid Texas driver’s license & must be insurable by Agency’s insurance.
Physical Requirements:  Ability to drive a vehicle; must have reliable transportation and a good driving record; must have a valid Texas driver’s license and must be insurable by Agency insurance; ability to communicate effectively; ability to pass the Preventative Management of Aggressive Behavior (PMAB) Training as well as all other physical requirements for this position.
