 Job Description: To provide immediate supervision, direction, instructions, and guidance over the day-to-day operations of the Community Living Options Information Process (CLOIP) and the Preadmission Screening and Residential Review (PASRR) Service Coordination Unit within the MHMR Authority of Brazos Valley service area of Brazos, Burleson, Grimes, Leon, Madison, Robertson, and Washington counties.  Service includes crisis prevention and management, monitoring, assessment, and service planning and coordination. As the Team Leader, serve as the point of contact for service coordinators working within the CLOIP/PASRR Unit, delegating tasks, making assignments, and monitoring program outcomes to ensure meeting of program performance targets, goals, and effective collaboration with state supported living centers, nursing facilities, other local intellectual and developmental disability authorities (LIDDA), private and public waiver providers, and other local and state agencies/entities.  Share caseload and provide service coordination for nursing facility residents who are eligible for (PASRR) Service Coordination within service area. Serve as Diversion Coordinator, fulfilling PASSR and LIDDA Performance Contract requirements for individuals with IDD related to nursing facilities.
Education:  Bachelor’s Degree from an accredited college or university with a major in social, behavioral, or human services field.Experience:  Requires at least one year of experience working with persons with intellectual or developmental disabilities. Prefer experience with prior casework or service coordination experience working with individuals with intellectual or developmental disabilities: must be computer literate, requires excellent verbal and written communication skills, requires excellent organization skills, including the ability to meet rigid timelines and manage multiple priorities.
Hours of Work:  Monday – Friday, 8:00 a.m. – 5:00 p.m.; some flexibility maybe required occasionally.
License/Certifications: Valid Texas drivers’ license and must be insurable by Agency’s insurance.
Physical Requirements:  Sight, hearing, ability to walk, ability to talk, lifting up to 20 pounds, ability to pass the Preventative Management of Aggressive Behavior (PMAB) Training as well as other physical requirements for this position.
