Salary: $32,454 to $39,550 Annually

 Job Description: As an ACT Team Recovery Facilitator this position provides crisis prevention and management, monitoring, assessment, rehabilitative skills training, service planning and coordination for adults with mental illness on the ACT Team. Psychosocial Rehabilitative Services are defined as social, educational, vocational, behavioral, and cognitive interventions provided by members of a client’s therapeutic team that address deficits in the individual’s ability to develop and maintain social relationships, occupational or educational achievement, independent living skills, and housing, that are a result of a severe and persistent mental illness. Service coordination involves accessing medical, social, educational and other appropriate services that will help an individual achieve a quality of life and community participation acceptable to the individual. Position requires on-call duties after hours and weekends on a rotational basis with other Team members. Some driving may be required within agency’s seven county service areas.
Education: Requires Bachelor’s Degree in a Behavioral, Social Science, Humanities or Education field.
Experience:  Requires experience working with individuals with mental illness, must be able to work independently as well as with a team. Good organizational skills are a must.
Hours of Work: 8:00am—5:00pm Monday—Friday; adjustment in work hours may be warranted to meet the needs of the program.
Licensure/Certification: Ability to be credentialed as a Qualified Mental Health Professional. Valid Texas driver’s license and are insurable by agency insurance.
Physical Requirements:  Ability to drive a vehicle; communicate effectively both verbally & written; ability to sit or stand for extended periods of time; Good computer skills a must; ability to pass the Preventative Management of Aggressive Behavior (PMAB) Training as well as all other physical requirements for this position.
