Salary: $32,454 to $39,550 Annually

 Job Description: To provide day-to-day coordination of services for adults and children with Intellectual Developmental Disabilities or a Related Condition enrolled into the Home and Community-based Services (HCS) Medicaid Waiver Program; to develop and monitor the Implementation Plan (IP) of personal outcomes developed from the Person Directed Plan (PDP); to develop and monitor the Individual Plan of Care, other pertinent assessments, and ensure continuity of services; make referrals for needed services; documentation of services provided/referrals made/interactions with providers, etc.; develop and engage in positive relationships with the HCS recipients/LAR/Advocate/Family Member, LIDDA Service Coordinator, community service providers, nursing and other professionals to assure the appropriate services are provided in a timely manner.
Education: Requires at minimum a Bachelors Degree in Human Services field, including but not limited to sociology, special education, rehabilitation counseling or psychology.
Experience: Two years’ experience with knowledge of HCS Medicaid Waiver Program is preferred; prefer experience working with persons with developmental disabilities or related conditions; knowledge of HCS rules, billing guidelines, standards and methods required; knowledge of habilitative and rehabilitative community based services; skill in determining individual needs assessment and the ability to respond to changes required; ability to comprehend and implement program goals; must be able to work independently as well as with a team. Good organizational skills are a must.
Hours of Work: 8:00am—5:00pm Monday through Friday; may require after-hours and weekends flexibility to meet the needs of the person in service/program.
Licensure/Certification: QIDP preferred. Must have reliable transportation and a good driving record. Must have a valid Texas driver’s license and be insurable by Agency insurance.
Physical Requirements: Ability to driver vehicle; ability to communicate effectively; ability to lift at least 30lbs.;  ability to pass the Preventative Management of Aggressive Behavior (PMAB) Training as well as all other physical requirements for this position.
Facility: Position will office out of Brenham and Bryan TX Offices.
