Salary: $37,598 to $45,819 Annually                   Salary is based on education & experience

Job Description: To provide crisis prevention and management, monitoring, assessment, and service planning and coordination for up to 45 adults and children with developmental disabilities or a related condition, enrolled into the Home and Community-based Services (HCS) Medicaid Waiver Program through local HCS providers in Brazos, Burleson, Grimes, Leon, Madison, Robertson, and Washington Counties. Service Coordination involves accessing medical, social, educational, and other appropriate services that will help an individual achieve a quality of life and community participation acceptable to the individual which is based on a Person Directed Plan (PDP). The Service Coordinator will negotiate and facilitate the array of services and support needed to address the individual’s personal goals and desired outcomes as identified through the Service Coordination Assessment and Service Planning Team. The Service Coordinator is responsible for ensuring planned services and support are implemented and appropriately documented. Will advocate and ensure consistent interactions with other agencies.
Education: Requires Bachelor’s Degree in behavioral, social science, criminal justice, humanities, or education field.
Experience: Prefer experience working with people with developmental disabilities or related conditions; must be able to work independently as well as with a team. Good organizational skills are necessary.
Hours of Work: 8AM-5PM Monday through Friday; may require after-hours flexibility to meet the needs of the person in service/program.
Licenses/Certifications: Must have reliable transportation and a good driving record. Must have a valid Texas Driver’s License and must be insurable by Agency insurance.
Physical Requirements: Ability to drive a vehicle; ability to communicate effectively; knowledge of case management principles, objectives, standards, and methods; knowledge of habilitative and rehabilitative community based services; skill in determining individual needs assessment and the ability to respond to changes required; ability to comprehend and implement program goals; ability to pass the Preventative Management of Aggressive Behavior (PMAB) Training as well as all other physical requirements for this position.
