Salary: $37,598 to $45,819 Annually                   Salary is based on education & experience

Job Description: Provides Intensive Case Management/Wraparound to children and youth and to their parents/LAR(s) for individuals with behavioral/emotional disorders as identified in the individual’s recovery/wraparound plan. Intensive Case Management includes assisting the individual with accessing medical, social, educational, and other appropriate services that will help the individual achieve a quality of life and family/community participation acceptable to the individual. Works with the family to identify a mission statement, identify unmet needs, and engage team members in the wraparound process. Works together with the identified Wraparound team to negotiate and facilitate the array of services and supports needed to address the individual’s personal goals and desired outcomes as identified through the assessment process. Also sees that the planned services and supports are implemented and appropriately documented. Coordinates treatment and advocates with other agencies and providers to assist the individual in reaching desired outcomes.
Education:
Requires Bachelor’s Degree in a behavioral, social sciences, humanities or education field; Must be able to travel out of town to attend required Wraparound Facilitator Training.
Experience: Prefers experience working with children and families; Requires knowledge of childhood development.
Hours of Work: Typically Monday—Friday; 8am—5pm; at times may need to have a flexible schedule to accommodate needs of program and families. Crisis on Call required as agency need dictates.
Licenses/Certifications:
Ability to be credentialed as a Qualified Mental Health Professional; valid Texas driver’s license and must be insurable by Agency insurance; Continued employment is contingent upon successful completion of Child and Adolescent Needs (CANS) certification.
Physical Requirements: Light lifting under 15lbs., moderate carrying 15-44lbs., simple grasping, walking, sitting, standing, operating a motor vehicle, operating office equipment including computer, ability to see and hear (with aid), ability to read, write, count, ability to maintain confidentiality, and the ability to work closely with others, ability to pass the Preventative Management of Aggressive Behavior (PMAB) Training as well as all other physical requirements for this position.
Facility: Children & Adolescent Services, Bryan, TX. or other service site as needed.
