Salary: $37,598 to $45,819 Annually                   Salary is based on education & experience

Job Description: Will serve as liaison with primary care providers in navigating individuals with co-occurring behavioral health and primary care conditions to appropriate services, which also includes providing professional therapeutic mental health services in accordance with Health and Human Services Commission (HHSC) Texas Resilience and Recovery (TRR) guidelines. Crisis on Call work required as agency need dictates.
Education:
Requires Bachelor’s Degree in a Behavioral, Social Science, Humanities or Educational field. Preferred: Master’s Degree
Experience: Prefer prior experience working with persons with major mental illness at a minimum. Prior experience working with primary health care preferred.
Hours of Work: Generally, Monday—Friday; 8am—5pm; at times may need to have a flexible schedule to accommodate needs of program and families.
Licenses/Certifications: Ability to be credentialed as a Qualified Mental Health Professional; valid Texas driver’s license and must be insurable by Agency insurance; Continued employment is contingent upon successful completion of Adult Needs and Strengths Assessment (ANSA) certification.
Physical Requirements: Light lifting under 15lbs., moderate carrying 15-44lbs., simple grasping, walking, sitting, standing, operating office equipment including computer, ability to see and hear (with aid), ability to read, write, count, ability to maintain confidentiality, ability to pass the Preventative Management of Aggressive Behavior (PMAB) Training as well as other physical requirements for this position.
Facility: Adult Mental Health or other primary care service site as needed.
